Service Description:

Group program running 3-5 days per week * assessment, stabilization, community referral and follow-up for individuals with a diagnosis of serious mental illness

groups include: Cognitive Behavioural Therapy, Health and Wellness, Mindfulness, Communication, Self-Esteem, Co-Dependency, Anxiety and Mood, Concurrent Disorders, and Dialectical Behavioural Therapy
length of program varies from four to eight weeks
individual counselling
family or social support counselling
brief transitional case management for those transitioning from the hospital to the community

Physician Referral Required (or Nurse Practitioner) - from inpatient unit, psychiatrists, Crisis Intervention Services and Emergency Department
