Service Description: The Addiction Supportive Housing Program is based on the "Housing First" model which provides supportive housing to people without requirements for engagement in addiction treatment or abstinence. The goal of the program is to increase the health and social outcomes of people with problematic substance use issues. The main objectives of the program are to reduce frequency of re-admissions to addiction programs, reduce contact with the criminal justice system, increase successful tenancy, and reduce emergency department visits. Primary eligibility requirements for the ASH Program include: - current problematic substance use and history of multiple entries in treatment continuum - unable to obtain or maintain housing without support - homeless or at risk of being homeless or marginally housed - willingness to identify harm reduction or abstinence substance use goals - capability and willingness to constructively participate in the ASH program. Successful applicants of the ASH program will receive a rent subsidy and an intensive case manager. Rent supplements are geared to a person's income as well as market rate for apartments.


Application: Referrals accepted from all sources, including self. Individuals must complete an intake and screening process to determine eligibility. Call, or walk-in to book an intake appointment. Telephone assessments can be an option in certain circumstances. Please call for an intake appointment.
