Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
Health care team includes doctors, nurse practitioners, nurses, dietitian, mental health case manager, health promotion workers * health care services include physical exams, care and treatment, referrals to specialists, counselling, and immunizations * disease and fall prevention services * health promotion activities
Offers pediatric care to children under the age of 6 who do not have a current family doctor or nurse practitioner
