Service Description: Provides services to in-patients, out-patients, and can conduct home visits to people in the communities * services include:
  • mobility
  • upper extremity rehab
  • acquired brain injury (ABI)
  • stroke
  • home modification
  • ADL management
  • amputee rehabilitation
  • provides consultation and ongoing treatment
  • will help to fill out forms

    Assisted Devices Program (ADP) authorizer for mobility and prosthesis

    Eligibility / Target Population: Members of the seven First Nations of the North Shore Tribal Council: Atikameksheng Anishnawbek (Whitefish Lake) First Nation, Batchewana First Nation, Garden River First Nation, Mississayga First Nation, Serpent River First Nation, Sagamok Anishnawbek First Nation, and Thessalon First Nation or the urban First Nations, Métis, and Indigenous people residing in the city of Sault Ste Marie
