Service Description: Supports and services for Indigenous people within the urban Indigenous community * helps people remain independent in their own homes for as long as possible * available to Indigenous members who are frail, vulnerable, at risk older adults, and adults who are disabled and chronically ill * includes nursing care, personal support, foot care, homemaking, meal preparation, and respite care
