Service Description:

Social, recreational, and therapeutic activities for people living with a chronic or disability in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
Additional services include:
- hot meals and snacks
- medication reminders and assistance
- assists with activities of daily living

Fees:

$12 per day includes meal and snacks * subsidy available for eligible participants * additional fee for transportation
