Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, registered nurse, dietitian, and social worker
Services:
- chronic disease education and prevention
- Well Women's Clinic (open to everyone)
- community outreach clinic (open to everyone)
- smoking cessation clinic
- mental health * one-on-one and group counselling * consulting psychiatrist
- prenatal and well baby clinic
- laboratory / diagnostics
- foot care clinic * specifically for people living with diabetes, with poor eyesight, arthritic hands, poor circulation, or poor sensation of the feet (open to everyone)
