As an Occupational Therapist (Permanent, Full-Time;
Effective: Immediately; Location: Sudbury), you are responsible for the provision of occupational therapy services to
Ontario Health atHome patients in order to maximize the patient’s function,
independence, and safety in their respective environment.
Whether you work in our office, in the
community, or a health care facility – you will play a lead role in providing
connected, accessible, patient-centered care – and be supported by our
collaborative team that includes over 9,000 regulated health care and other
professionals.

What will you
do?

Assesses the
factors influencing the patient’s ability to function in their environment,
using a variety of standardized and non-standardized assessment tools.
·
Determine
patient centered therapy goals and provides treatment program to increase
occupational performance, independence, and safety.
·
Share knowledge
and information with other health care professionals and agencies to work
collaboratively to deliver effective and efficient patient care.
·
Builds
therapeutic relationships by ensuring patients’ values and preferences are
respected and balancing patient needs and choices with available resources.
- Documents assessment findings, goals, and
intervention plan in accordance with Ontario Health atHome policies and
the College of Occupational Therapists of Ontario standards for record keeping.
What must you
have?
- Registered
Occupational Therapist in good standing with the College of Occupational
Therapists of Ontario.
- In-depth
knowledge of occupational therapy services and other community resources.
- Good
understanding of the roles of health care professionals.
- Relevant
occupational therapy experience in a community/health care environment.
- Comprehensive
and up-to-date knowledge of occupational therapy tools, processes,
equipment, and assistive technology.
- Knowledge
of funding agencies related to the occupational therapy recommendations.
- Practical
knowledge and understanding of relevant legislation (e.g. regarding the
provision of health care services, privacy, health, and safety, etc.).
- Comprehensive
knowledge of the standards of practice and professional guidelines set
forth by the College of Occupational Therapists of Ontario.
- Ability
to prioritize professional duties, manage multiple patients, and
efficiently organize workload.
- Strong
interpersonal and collaboration skills to work with diverse patient
groups, case managers, family members/caregivers/Substitute Decision
Makers, health care professionals, community organizations and service
providers.
- Effective
listening, observation, and facilitation skills.
- Ability
to communicate information effectively through a variety of means
including reports, letters, meetings, and presentations.
- Working
knowledge of computer software (email, internet) and Microsoft Office
applications (Word, Excel).
- Must have valid driver’s license
and access to a vehicle, as regular travel within the Temiskaming District
will be required. Travel to other areas within the North East geography
may also be required from time to time, as the business needs require;
- Advanced
written and oral proficiency in English is essential.
