As a Physiotherapist (Status: Part-Time; Effective: Immediately; Location: Sault Ste.
Marie), you provide physiotherapy services to Ontario
Health atHome patients to reduce, eliminate, or prevent physical
disabilities/impairments and improve the patient’s functional independence and
safety in their respective environment.
Whether you work in our office, in the
community, or a health care facility – you will play a lead role in providing
connected, accessible, patient-centered care – and be supported by our
collaborative team that includes over 9,000 regulated health care and other
professionals.

What will you do?

Respond to patient referrals from Care Coordinators to properly
administer physiotherapy services.
·
Assess
patient's physical abilities to determine the impact of the patient’s condition
on their ability to function in their environment.
·
Analyze, interpret, develop, and implement treatment programs
based on assessment results.
·
Oversee patients progress and adjust treatment programs when
needed, to ensure therapy goals are met.
·
Assign physiotherapy programming to a rehabilitation assistant
based on patient needs and supervise according to professional standards.
·
Educate and train patients, their families, caregivers, and/or
SDMs, on how to progress with the treatment plan.
·
Link patients with other in-house services or outside
organizations if needed.
·
Balance patient needs and choices with available resources,
ensuring patients’ values and preferences are respected.
·
Accurately document and chart patients’ assessment results, goals,
treatment plans, and interventions.
What must you
have?
- Registered
Physiotherapist in good standing with the College of Physiotherapists of
Ontario.
- In-depth
knowledge of physiotherapy services and other community resources.
- Good
understanding of the roles of health care professionals.
- Relevant
physiotherapy experience in a community/health care environment.
- Comprehensive
and up-to-date knowledge of physiotherapy tools, processes, equipment, and
assistive technology.
- Knowledge
of funding agencies related to the physiotherapy recommendations.
- Practical
knowledge and understanding of relevant legislation (e.g. regarding the
provision of health care services, privacy, health, and safety, etc.).
- Comprehensive
knowledge of the standards of practice and professional guidelines set forth
by CPO.
- Ability
to prioritize professional duties, manage multiple patients, and
efficiently organize workload.
- Strong
interpersonal and collaboration skills to work with diverse patient
groups, Care Coordinators, family members/caregivers/Substitute Decision
Makers, health care professionals, community organizations and service
providers.
- Effective
listening, observation, and facilitation skills.
- Ability
to communicate information effectively through a variety of means
including reports, letters, meetings, and presentations.
- Working
knowledge of computer software (email, internet) and Microsoft Office
applications (Teams, Word, Excel).
- Must
have valid driver’s license and access to a vehicle.
- Advanced
oral and written proficiency in English is essential.
- We have a
