Service Description: Support for people living with prediabetes or diabetes * one-one-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Outreach via Ontario Telemedicine Network (OTN) to the following nursing stations: Rosseau, Point au Baril, Whitestone, Britt, Argyle, and Moose Point

Application: Call * no referral required * drop-in service Friday mornings only
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 and type 2 diabetes and gestational diabetes
