Service Description: Provides a range of palliative care support for terminally ill individuals and for their families * provides the following services:

Residential Care:
  • 24 hour nursing and personal support
  • homelike environment with 10 private rooms
  • comprehensive pain and symptom management
  • supportive care and complimentary therapies
  • pediatric admissions are on a case-by-case admission process

Bereavement Services:
  • emotional, social and spiritual support to individuals facing death and dying and their families
  • one-on-one discussion for the resident and/or family
  • bereavement support groups

Supportive Care:
  • support for spiritual/religious rituals
  • support for issues regarding death and loss
  • support for end-of-life decision making

Outreach services ARCH Connect:

Built on a collaborative shared model approach * outreach Nurses work with the primary care team to assess and plan interventions that address the physical, social, emotional, and spiritual needs of patients and their families

The Connect team positively impacts the experience of life-limiting illness and ease the transitions through proactive and comprehensive care planning, timely therapeutic intervention, and coordination of care

Eligibility / Target Population: Adults and paediatrics who have been diagnosed with a terminal illness and have an estimated prognosis of 3 months or less
Languages: English
Language Notes: Translation, bilingual service and support in some languages are available
