Service Description: Provides rehabilitation workers to assist adults with acquired brain injuries in their community or home individualized or group community-based rehabilitation to promote independence in various areas of day to day functioning (physical, cognitive, behavioral, social, activities of daily living)

Eligibility / Target Population: Older adults and people with disabilities, who need physical, social, or emotional supports
