Service Description: The role of the Psychogeriatric Resource Consultant (PRC) it to support healthcare providers in long-term care homes (LTCHs), Home and Community care, hospitals, and MOH funded community support organizations that provide care for older adults with or at risk of, responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions.

Application: The focus of the program is on the provision of comprehensive health assessments, and individualized person-centred interventions to support the individual and their formal care partners. Facilitating consultations with specialty care providers and offer enhanced transitional supports across sectors. The referral process is by phoning or filling out an electronic referral form online to the North East Behavioural Supports Ontario.
