Service Description:

Case Management for pregnant or parenting (children under the age of six) women or men provided on both an individual and occasional group basis.

Self-referrals accepted
Call, email or complete the online referral form.
Intake can take place by phone or in the client's home.
The applicant must agree to community case management. If the individual wishes to proceed following the assessment and information session, individuals will be assigned to an outreach worker, who will begin services within the week.
