Service Description: Helping those who have had a stroke to get back to daily activities and live the fullest life possible * include programs such as:

  • living with stroke
  • together in movement and exercise
  • fitness and mobility exercise programs
  • cognitive remediation
  • assessment services
  • monthly support group/peer group
  • individually designed physical therapy programs
  • attendant services
  • supportive housing

  • Fees: Free
    Application: Call * no referral required
