Service Description: Supports for the activities of daily living such as:
  • living with stroke education and support group
  • peer support program
  • assessment services
  • cognitive remediation
  • heart healthy kitchen program
  • horticultural therapy
  • many exercise programs available
  • aqua fitness
  • outreach attendant services who visit in the home to assist with many activities of daily living
  • access available for post stroke transitional care unit when transitioning from hospital to home, for short respite stays or for independent living assessments

  • Fees: Free * fees for transitional care unit
    Application: Referral from North Bay Regional Health Centre (NBRHC)
    Eligibility / Target Population: Adults and youth 16 years and older, and their families living with the effects of a stroke * valid health card required
