Service Description:
|
Provides health services in school for students who require medical and/or rehabilitative assistance to enable them to attend school, participate in school routines and receive instruction
Services available to eligible patients may include:
- personal support
- nursing
- nurse practitioner
- physiotherapy and occupational therapy
- social work
- nutrition and food counselling (dietitians)
- access to long-term care homes
- access to supported living (adult day programs, supportive housing, retirement homes and assisted living)
- health support in the school
- medical supplies and equipment
- speech and language therapy
- referral to other community supports
In addition to delivering care in schools, visiting professionals will also provide health-related education, teaching and consultation services to school staff and parents in the school setting
