Service Description:
|
Provides assistance with some of the travel and accommodation related costs for Northern Ontario residents who must travel at least 100 kilometres one-way to access medical specialist or designated health care facility services that are not locally available

Application:

Must be referred by a northern Ontario health provider (physician, dentist, optometrist, chiropractor, midwife or nurse practitioner)

Eligibility / Target Population:

Application form must be filled out by you, the referring Northern health care provider and the specialist or designated health care facility)- Must have a valid Ontario health care
- Health care provider must be a ministry designated facility
|
