Supports for people living with pre-diabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, certified diabetes educator/dietitian and foot care nurse on staff- diabetes

- offers on going treatment and consultations
- weight management
- insulin adjustments
Offers self management workshops for:
- diabetes
- chronic diseases
- pain management
- foot care
Outreach clinics provided in: Matachewan, Matachewan reserve, Kirkland Lake Canadian Mental Health Association and Family Health Team, Larder lake, Virginia town- 1 week waitlist
- assists patients to access funding (forms)
