Service Description: Health centre providing family medical care and community-based health programs * team of family doctors, nurse practitioners and other health care providers * referral to other regional health services

Culturally appropriate programs and services for Indigenous people, combining traditional health and Western medical practices * aims to improve the social supports and conditions that affect long-term health

Smoking cessation programs (helps to quit smoking), home and community care program

Health Education and Promotion - Walk-in clinic, mental health counselling, care coordinator and referrals to other agencies if required, wellness groups for men and woman, traditional ceremonies for health issues, information about traditional herbs and medicines and consultations with traditional healers, prenatal and postpartum health care, clinics on issues related to diabetes, immunization, sexual health and communicable infections, fetal alcohol syndrome, fetal alcohol effects

Walk-in medical clinic - Nurse practitioner and nurses assess and treat minor illnesses and injuries

Hospital Discharge Program - N'Mninoeyaa Aboriginal Health Access Centre - Mamaweswen, North Shore Tribal Council - Nurse case managers provide support to older adults and adults with disabilities who require long term care and personal support after regular work hours and on weekends * coordinates community care services to meet at-home health care needs upon discharge from hospital * includes personal support, homemaking, and respite services
