Service Description:
|
Provides students with an intellectual disability, a
transition plan that is person-centered and used to facilitate a smooth and
successful transition from school to community life * transition coordinators can
assist high school students and their families to plan for their future and to
set personal goals.
Services provided:
- facilitates transition from school to community life through a self-directed
transition plan
- coordinates plans to support personal goals
- provides opportunities to build connections within the community
- assists with applications for funding and referrals to community services
- collaborate and consult with local high schools
