Service Description:

Nurse practitioner works with a social worker and other health professionals to assess and treat non-life threatening injuries or illnesses
Chronic disease prevention and management program - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions * for diseases such as diabetes, arthritis, asthma, heart disease
Other services:
- immunization clinic (vaccination against common infectious disease)
- fall flu immunization clinic
- smoking cessation
- nutritional education
- annual physicals
- counseling
- health promotion
- illness preventon
- falls prevention program for older people
