Youth and Family Engagement for Intervention Survey

The Centre for Addictions and Metal Health in partnership with the Service Collaborative invites you to participate in a brief survey.

What is the purpose of this survey?

  • The Service Collaborative is a group of people from Sudbury, Little Current and Chapleau that work with youth who have addictions and mental health concerns. They
    get together every month to make services better for youth, their families and loved ones.
  • The Sudbury-Manitoulin Service Collaborative is surveying youth (16 years of age or
    older) and their families to help them pick a new model for helping youth.
  • The purpose of the online survey is to hear the perspectives and opinions of youth and their families on what model they feel will be the most useful/impactful to help the Sudbury-Manitoulin Service Collaborative.

 

As a participant, you will be asked about which intervention you feel would best help you.

This survey is totally voluntary and anonymous.

It will take approximately 10 – 15 minutes of your time.

The results of the survey will be shared with Sudbury/Manitoulin Service Collaborative to help them decide which model to implement in the region.
