Service Description: Acquired Brain Injury Outreach Services - Community programs and support for people with physical disabilities or those with an acquired brain injury offering assistance in enhancing independence and community participation

Post Stroke Transitional Care Services - Services include community navigator to assist individuals as they are being discharged from the hospital in addition to those needing assistance in the community * rehabilitation supports are also available to support enhancing physical and cognitive skills
